The shifting of gfn seems to happen bother here and in the callers.
Sorry. I overlooked this line. I can add another preparatory patches for callers to pass in an IOVA other than "pfn << PAGE_SHIFT", if you think it's necessary: although GVT still does things in PFN, both ap and ccw prepare their PFN lists from IOVAs, which now can be omitted.
